To Select a Donor:

1. View Donors
   (NOTE: Password is "view")
    

2. Provide your fertility center the Donor ID and have them call us. 

   • Sorry, patient calls not accepted.

   • Please respect that.

   • Have your fertility center call:
     (513) 769-5259  M-F 9-5 EST
      

3. We will convert the entire online application to a PDF file and send it to them.  This provides them the contact information for your donor.
    

4. The fertility center takes over from here.

There is no fee for this service, likely saving you $3000 to $5000 in agency fees if everything works out.  We wish you the best.

Fertility Network provides many opportunities to link up with our customers in many locations throughout the USA.  Where we don't have a customer that we support, we provide the opportunity to donors to apply through "Fertility Network Agency". 

About our donors:

• We have been recruiting donors in the USA since 1998.
• We support customers in over 100 markets in the USA.
• Our donor eligibility processes are often duplicated by fertility centers as a model through "DonorApplication.com".
• The donors presented to recipients have passed a basic pre-screen, an FDA based survey, and a thorough overall donor application whereby it has been determined that they likely will pass the remaining steps within the qualifications of a fertility center.

Within the profiles are two columns "2nd City" and "Extra $2000".  The questions on the application provide you with insight.  They are not part of a binding contract with the donor.  Here is what they mean:

• 2nd City - Would the donor be willing to travel to the next closest city for your treatment?
• Extra $2000 - Would the donor be willing to travel throughout the USA for an extra $2000 to cover expenses?
