Pre-Qualification Form to Donate Eggs

	1.	I am a non-smoker.
	2.	I am between the ages of 21 and 30.
	3.	I have significant knowledge of my biological family.
	4.	I have not had tattoos or piercings within the past 6 months at a facility that did not use sterile procedures.
	5.	I do not have a sexually transmitted disease.
	6.	I live in the Central Florida area.
	7.	I realize to donate eggs is an involved process requiring medical and psychological screening, with processes required by the FDA for human tissue management.
